“DNA Mobile Testing Services” by Uniqstic Health Policies and Procedures.

Mileage Policy:

• • Standard Mileage Rate: A mileage fee of .67 cents … will be charged beyond a 20-mile radius from the service provider’s location.
  • Extended travel: For distances over 30 miles, additional travel time may be incurred at an hourly rate of …$50 per hour (subject to change depending on distance)
  • Calculation: Mileage is computed from the starting point of the appointment location and back to the starting point 

Same Day appointment fees

• • Urgency fee: Same-day appointments can be accommodated but will incur a fee of $25 based on availability.
  • Booking Window: Same-day appointments must be booked at least 2 hours in advance. Requests made after 3 pm will incur an additional fee of  $15.
  • Availability: Same-day appointments are not guaranteed and depend on the availability of resources and staff. Same Day fee: $25
  • Payment: The fee must be paid in full at the time of setting the appointment

Additional Parties

• • Base fee coverage: The testing fee includes up to or no more than two parties, father/mother and a child.
  • Additional party fee; For each additional party could be a child or mother, there will be a fee of $150 applied.
  • Group Discounts: promotions are offered for more than four parties. Please reach out via email for a quote.
  • Documentation: At the time of the meeting all forms and identifications must be provided

Out Of State

• • Base fee for out of State Collection..$79 on top of the standard collection rate that is set if customers were residing within the service State.    

Refund Policy

• • Refund Eligibility: Refunds will be issued if a cancellation request is received 24 hours before the scheduled appointment.
  • The $80 Deposit due prior to the collection appointment will only be refunded if the reason for cancellation is verified and approved by Uniqstic Health.
  • Processing time: Any refunds issued will be processed within 2-5 days of the day it’s received.
  • Non-Refundable Fees: The following fees are non-refundable: same-day, mileage, and any miscellaneous fees.
  • Partial Refunds: if any test is performed and not completed, a partial refund might be issued excluding fees for services rendered.

Cancellation Policy

• • Standard Cancellation: Cancellations must be done at least 24 hours before the scheduled appointment without being charged a fee or penalties
  • Late cancellation: Cancellations made within 24-48 hours of the appointment will incur a cancellation fee of $25
  • No-show policy: Failure to appear for the set appointment without cancellation will hereby result in the client being charged the full amount of the test being requested and refunds will not be given.
  • Rescheduling: Appointments can be rescheduled once without penalty if a written and/or verbal notice is received in advance
